Why Acid Reflux Causes Chest Pain (and When It's Not Reflux)
Luke McKnight, Founder, GERDBuddy
First: call 911 if
- The pain is new, crushing, or feels like pressure or squeezing
- It spreads to your arm, jaw, neck or back
- You're short of breath, sweaty or lightheaded
- You feel sick to your stomach with it
You can't reliably tell heart pain from reflux pain on your own, and neither can doctors without tests. That's why emergency teams check the heart first, every time. Women in particular sometimes delay care because they assume a heart attack is heartburn.
Everything below is for people whose heart has already been checked.
Why reflux can feel like your heart
Your esophagus runs right behind your heart. It passes through the middle of your chest, so acid irritating it is felt in the same spot.
They share the same wiring. Nerves from the esophagus and the heart enter the spinal cord at overlapping levels. Your brain gets the pain signal from roughly the same place and can't easily tell which organ sent it. That's how a burning esophagus can register as a squeeze.
It's one of the most common non-heart causes. Once the heart is ruled out, reflux is among the most common explanations left for chest pain. Plenty of people go through a full cardiac workup before learning it was their esophagus.
Signs that lean toward reflux
These point toward reflux, but they never rule out your heart:
- A burning feeling behind your breastbone
- It shows up after big meals or when you lie down
- A sour or bitter taste comes with it
- It gets worse when you bend over
What helps once your heart is cleared
Finish dinner 3 hours before bed. In one large study, people who went to bed within 3 hours of dinner were about 7 times as likely to have GERD as those who waited 4 or more hours.
Raise the head of your bed 6 to 8 inches. Use risers or a foam wedge so acid has to travel uphill. Stacked pillows bend you at the waist and can squeeze your stomach.
Keep meals smaller. A stretched stomach makes the valve at its top relax more often, which lets more acid up. See why acid reflux burns for how that valve works.
Track your flares. Once your doctor says it's reflux, log when chest pain shows up and what you ate before. GERDBuddy turns those logs into patterns, so you can see which meals and times keep setting it off.
